Transformers Age of Extinction 2014 1 — 1. Transformers: Age of Extinction (2014)

Michael Bay normally pays professionals to send large metal objects flying through the air. During the Hong Kong shoot for *Transformers: Age of Extinction*, however, a confrontation erupted after a local shop owner allegedly demanded HK$100,000 from the production. Police and Paramount offered slightly different accounts of the melee, but both involved assaults, arrests and a man reportedly swinging an air-conditioning unit at Bay’s head. Titanic 1997 1 — 2. Titanic (1997)

On paper, *Titanic* already had enough disasters: a runaway budget, a punishing water shoot and a ship whose ending was famously non-negotiable. Then a midnight meal in Halifax sent roughly 80 cast and crew members to hospital after someone laced the lobster chowder with PCP. James Cameron and Bill Paxton were among those affected as police opened an investigation into the catering operation. Fast Furious 9 2021 — 3. Fast & Furious 9 (2021)

*Fast & Furious 9* treats broken laws of physics as family entertainment, but gravity became brutally real for stunt performer Joe Watts. After a stunt was altered, his safety line detached and he fell approximately 25 feet onto concrete at Warner Bros. Studios Leavesden, suffering a serious brain injury and other life-changing damage. Investigators found inadequate crash matting and an overdue harness inspection. Jumper 2008 1 — 4. Jumper (2008)

Teleportation carried *Jumper* across the world, but the production left behind a tragedy on one of its Toronto sets. While workers dismantled a 30-foot exterior structure, frozen sand, gravel and ice collapsed onto veteran set dresser David Ritchie, killing him instantly and seriously injuring another crew member. Jumper Productions later pleaded guilty to three violations of Ontario’s workplace-safety law and received a C$250,000 fine. Cropped The Crow — 5. The Crow (1994)

Brandon Lee was filming the scene in which Eric Draven is murdered when a prop revolver turned fatally real. A projectile left behind by an improperly prepared dummy cartridge had become lodged inside the gun’s barrel; when a blank was fired during the next setup, it propelled the obstruction into Lee. The actor died hours later at 28. Investigators blamed negligence in the handling of the weapon, although no criminal charges followed, and *The Crow* was eventually completed using doubles and visual effects. | © Miramax Films

Wild Things 1998 — 6. Wild Things (1998)

A floating corpse sounds exactly like something *Wild Things* would introduce before revealing three betrayals and a suspiciously convenient insurance policy. This one was not in the screenplay. While filming in the Florida Everglades, Kevin Bacon and other crew members spotted a body drifting through the marsh, forcing production to stop while authorities recovered it. The body was removed before it floated into the shot, and no confirmed cause of death was publicly disclosed—so the discovery remains unsettling, but should not automatically be labeled a murder. | © Columbia Pictures

Cropped Cannibal Holocaust 1980 — 7. Cannibal Holocaust (1980)

*Cannibal Holocaust* looked so authentic that authorities seized the film and suspected director Ruggero Deodato had recorded genuine human killings. Deodato’s defense required a reveal worthy of the movie: he produced the supposedly dead actors in court and explained how its most infamous effects had been constructed. The performers were alive, and the human carnage was staged. The animal killings captured on camera, unfortunately, were real—a distinction that rescued Deodato from murder allegations while ensuring the film’s notoriety would outlive every censorship battle surrounding it. | © F.D. Taxi 2 2000 — 8. Taxi 2 (2000)

The car chase that went wrong in *Taxi 2* was supposed to send a vehicle sailing over two military tanks and onto a prepared landing area. Instead, the stunt car traveled beyond its mark and struck cameraman Alain Dutartre and his assistant, Jean-Michel Bar. Dutartre died from his injuries, while Bar suffered severe damage to both legs. Years of legal proceedings followed, with an appeals court finding production company EuropaCorp—then operating as Leeloo Productions—guilty of involuntary manslaughter over the fatal stunt. | © Leeloo Productions

Twilight Zone The Movie 1983 — 9. Twilight Zone: The Movie (1983)

A nighttime sequence for *Twilight Zone: The Movie* called for Vic Morrow to carry child actors Myca Dinh Le and Renee Shin-Yi Chen through a simulated wartime attack. Explosions damaged a low-flying helicopter, which crashed onto the performers and killed all three. The children had been working after permitted hours without the required authorization, compounding questions about how the scene was organized. Colors 1988 — 10. Colors (1988)

Sean Penn supplied *Colors* with an unscripted outbreak of street violence during a filming break at Venice Beach. After extra Jeffrey Klein photographed Penn and Robert Duvall without permission, an argument escalated through insults, spitting and several punches to Klein’s face. Penn was already on probation, and the assault—combined with a separate reckless-driving offense—earned him a 60-day jail sentence. Rust 2025 — 11. Rust (2025)

The rehearsal inside *Rust*’s wooden church set was not supposed to involve gunfire. A revolver held by Alec Baldwin discharged a live round, killing cinematographer Halyna Hutchins and wounding director Joel Souza. Armorer Hannah Gutierrez-Reed was later convicted of involuntary manslaughter, while assistant director David Halls pleaded no contest to negligent firearm handling. Clownhouse 1989 — 12. Clownhouse (1989)

The most horrifying story attached to *Clownhouse* took place away from its masked villains. During production, director Victor Salva sexually abused 12-year-old leading actor Nathan Forrest Winters and recorded one of the encounters. Salva confessed, was sentenced to three years in prison and served approximately 15 months. The film may have introduced audiences to a young Sam Rockwell, but its legacy remains dominated by the crime against Winters—and by the uncomfortable fact that Salva was later allowed to resume a substantial filmmaking career. | © Zoetrope Studios

Maidstone 1970 — 13. Maidstone (1970)

Norman Mailer designed *Maidstone* as a deliberately unstable collision between performance, politics and improvisation. Rip Torn pushed that experiment past its safety rail when he suddenly struck Mailer on the head with a hammer while the cameras were still running. Mailer fought back, biting into Torn’s ear as blood flowed and horrified family members looked on. Les Volets Verts 2022 — 14. Les Volets Verts (2022)

*Les Volets Verts* cast Gérard Depardieu as an aging screen legend confronting the wreckage hidden beneath his public mythology. Off camera, two women working on the production—a set dresser and an assistant director—said the actor sexually assaulted them during filming. A Paris court found Depardieu guilty and imposed an 18-month suspended prison sentence, along with additional penalties. He denied the allegations and announced an appeal, but the verdict marked the first criminal conviction in the actor’s widening series of sexual-misconduct cases. | © ARP Sélection

Midnight Rider canceled movie — 15. Midnight Rider (Canceled)

*Midnight Rider* never reached theaters; its first day of filming became the event for which it is remembered. The crew placed a hospital bed on an active railway trestle despite lacking permission to shoot there. When a freight train arrived, camera assistant Sarah Jones was struck and killed, while seven other crew members were injured. Director Randall Miller pleaded guilty to involuntary manslaughter and criminal trespass, served a year in jail and later completed probation. In several cases, the courtroom aftermath lasted longer than filming; in others, nobody was ever held responsible. The blood was no longer stage dressing, and the police tape definitely was not part of the production design.
