The Milano 821 was completely neglected until it received a buff during Season 4 of BOCW, in the late summer of 2021, and since then, it's been far stronger than it has any right to be. This is the first SMG you unlock in BOCW, it's the rubbish level 1 weapon you get while you're just starting to grind. No one expects this kind of weapon to be good. But after the buffs, which gave it great damage over range, the Milano can compete with the best Sniper Supports in the game, and is maybe the single best SMG in this role. The 3x might seem a little extreme, and there are good arguments to be made for the 2x or even the Microflex LED, but with this optic you can really take advantage of the Milano at ranges from the 30-50 m range. With every other attachment we're focusing on good recoil control, lots of firepower, and as much extra range as we can get.

For the Milano, we're pushing it into the Sniper Support role, so obviously the best secondary weapon is a Sniper. And we've chosen our current favorite: the ZRG 20mm. But you could use a long-range AR like the STG or the BAR with the Milano, if you don't like sniping. This ZRG setup has been pretty standard for a couple of weeks now, and is the currently strongest sniper in the game.
The Best Equipment For A Milano 821
Stims are now a great offensive option in Warzone Pacific because of the great speed boost that they offer, so we'll take them for the pushes. And Throwing Knives allow us to eliminate downed players quickly while we're in close combat, so also great for aggressive pushes.
The Best Perks For A Milano 821
If we're sniping, then Amped is great because it allows us to switch to the Milano as quickly as possible if detect a threat getting close. E.O.D. is just a great passive defense buff that comes in handy in most games, and Overkill is obviously required in a Sniper/Sniper Support loadout.
